My Philosophy and Approach to Wellness...

Family medicine is about families. My desire as your family physician is to provide you with the same type of personalized healthcare that I expect for my own family. As an MDVIP-affiliated primary care doctor, I have the time to focus on your healthcare needs and those of your loved ones. By providing a warm, unhurried atmosphere, I have the energy to serve as your primary care physician in a manner that you deserve. It will be an honor, as your personal doctor, to develop the type of relationship that fosters excellent health and well-being across the life spectrum.

Apart from obstetrics, I practice the full scope of family medicine, from birth through the type of end-of-life decisions that my additional board certification in Hospice and Palliative Care necessitate. I believe in quality of life throughout an entire lifetime and will work as a team with you and yours to ensure that you are living your ideal life. My MDVIP-affiliated practice offers services, including comprehensive, advanced health screenings and diagnostic tests, that go far beyond those found in concierge medicine practices. It is my fervent desire to know my patients well and, by doing so, be better equipped to manage their health.

I have special interests in pediatric, adolescent, adult and geriatric health, chronic disease management and minor office procedures, to name a few. I suppose that doesn't really narrow it down, but in my office we will be able to focus on those interests that are important to you, with an emphasis on wellness and prevention. I am fluent in English and Spanish, a member of the Bayhealth Board of Directors and Medical Staff and serve Central and Southern Delaware and beyond!

I look forward to our future together.

Telemedicine Option: I am also using telemedicine visits to stay connected with my patients when they are unable to see me in the office. It is easy to do, and we have maintained our momentum with preventive health goals and chronic care during difficult times.

Four years ago I developed a lump in my left breast. Going back 25 yrs. I developed breast cancer in the left breast. Had a lumpectomy, 4 rounds of chemo and 33 sections of radiation. Never have I missed a mammogram since 1996. The last 4 years always received a letter from Bayhealth Radiology saying that I have dense breasts. The decision to become an MDVIP patient was a tough one. Being a senior on a fixed income, the question regarding whether or not to pay for membership to continue to see my doctor was a challenge, but when that doctor is Scott Hammer, the answer became pretty easy. My relationship with Dr. Hammer goes back to 2012 and I've always held him in high esteem. Joining him at MDVIP allowed us… See more

Last October I was in a car accident out of state and had experienced sudden onset of hearing loss from an airbag exploding. When no one in the emergency room took it very seriously, between my daughter, who is an out-of-state ENT and Dr. Hammer a decision was made to prescribe high dose prednisone for the hearing issue. I found out later that this is essentially an emergency situation, so I’m… See more

When Dr. Scott Hammer delivered the news of my husbands cancer diagnosis. His personal concern for us, understanding the shock of the news, the processing necessary for quick decisions to be made in order to facilitate a treatment plan was profound for us. His professional guidance, the quick actions of his staff to facilitate needed outside intervention, tests, and schedule them for us was… See more

Several weeks ago I developed lower back pain, which progressively got worse as the day went on. By the next morning my left leg had lost all of its strength. Being a MDVIP member I was able to contact Dr. Hammer before office hours on his personal cell phone. That is something that doesn’t happen in the alternative medical world. I was in his office by 10 AM that morning and Scott was able to… See more

Both my wife and I are patients of Dr. Scott Hammer in Milford Delaware. This past summer we travelled to Colorado with another couple and spent two weeks exploring the Rocky Mountain state. After a couple of days the first signs of Covid hit. First our friends and then my wife and myself. Once we tested and verified, I sent a text to Dr. Hammer. He responded immediately. I told him… See more

When my 93 year old Dad was recovering from Colon Cancer surgery we met Dr Scott Hammer. As we were leaving his office my Dad turned to me and said "I've been going to Doctors for 90 years and when I finally find one that I like he looks 30 years old and has a pony tail". The most memorable moment occured a couple of years later as the cancer had metastasized and was now in his… See more
