MDVIP maintains internal logs for Internal Protocol (“IP”) addresses. An IP address is a number that is automatically assigned to your computer each time you access the Internet. Your IP address does not contain personally identifiable information. MDVIP collects IP addresses in order to conduct system administration, report aggregate information, conduct site analysis, and to detect improper use of the site.

We use “cookies” to collect information on how our Site is used. By placing a small file known as a "cookie" on your computer (or other device), MDVIP, and its third party service providers’, servers passively gather information about all visitors' use of the Site for several reasons, including, but not limited to, the following: statistics collection and analysis, Site optimization, analytics, market research, and maintenance of user login information. The information that we and our third party service providers track with cookies includes, but is not limited to, the type of browser (such as Google Chrome or Internet Explorer) and Internet-connected device being used to access the Site, your IP address, your home domain or Internet service provider, your referrer URL (which is the URL for the Site that you were viewing prior to visiting the Site), how you were directed to the Site, which specific pages you access on the Site, how long you view each page, the time and date you access our Site and the total number of visitors to the Site and any portions thereof. We use your IP address to determine the general physical location of your computer or device and understand from what geographic locations our Site visitors come.

We, and/or our third party service providers, may place cookies or similar files on your computer for security purposes (such as authentication), to facilitate site navigation and to personalize your experience while visiting our Site. Cookies allow us and our third party service providers to collect technical and navigational information, such as browser type, time spent on our Site and pages visited (these types of cookies are typically referred to as “Session Cookies”).

Most popular Internet browser applications will allow you to configure the browser so as not to accept cookies. However, setting your browser to reject cookies may, in certain instances, prevent you from taking full advantage of the Site and its capabilities, or cause some features of the Site to function improperly or more slowly.

The Site uses Google Analytics (including the Universal Analytics feature), a web analysis service provided by Google, in order to better understand your use of our Site and how we can improve the Site. Google Analytics collects information such as how often users visit a Site, what pages they visit when they do so, and what other websites they used prior to coming to such Site. Google Analytics collects only the IP address assigned to you on the date you visit the Sites, rather than your name or other identifying information. We do not combine the information collected through the use of Google Analytics with your personal information. Although Google Analytics plants a permanent cookie on your web browser to identify you as a unique user the next time you visit a Site, the cookie cannot be used by anyone but Google. Google’s ability to use and share information collected by Google Analytics about your visits to the Site is restricted by the Google Analytics Terms of Use and the Google Privacy Policy. Google utilizes the data collected to track and examine the use of the Site, to prepare reports on its activities and share them with other Google services. Google may use the data collected on the Site to contextualize and personalize the ads of its own advertising network. To more fully understand how Google may use the data it collects on the Site, please review “How Google uses data when you use our partners' sites or apps”, (located here or any successor URL). If you would like additional information regarding the security and privacy practices of Google’s Universal Analytics feature, please review the materials provided by Google here.

While we make every attempt to protect the personal information that you share with us, it is important to note that an e-mail message is unsecured. Should you choose to send your information via e-mail, you do so at your own risk. We recommend that if your communication is extremely sensitive you may want to send it by postal mail.

If you choose to correspond to us via e-mail, we reserve the right to retain the content of your email messages as well as your e-mail address or other personally identifiable information, and our responses.

If you include personally identifiable information as part of any content that you provide to MDVIP via public postings to the site, including any bulletin boards, forum posts, etc., such information may be made available to any parties using the site. MDVIP does not edit or otherwise remove such information from content uploaded to any bulletin boards, forum posts, etc. before it is posted on the site. If you do not wish to have your personally identifiable information made available in this manner, you must remove any such information from the content before posting. Your contact information (which may include, without limitation, a username and email address) may be included with any content that you post via the site. MDVIP is not liable for any damages caused or incurred due to personally identifiable information made available in the foregoing manner.

We are not responsible for, and this Privacy Policy does not apply to, the privacy practices of those sites or of any companies that we do not own or control. We make no representations or warranties regarding the correctness, accuracy, performance, or quality of any content, software, service, or application found at any Linked Site. We are not responsible for the availability of the Linked Sites or the content or activities of such sites. If you decide to access any Linked Site, you do so at your own risk. In addition, should you happen to initiate a transaction on a website that our Site links to, even if you reached that site through the Site, the information that you submit to complete that transaction becomes subject to the privacy practices of the operator of that Linked Site. Your use of any Linked Site is subject to the policies, terms of use, and privacy policies of such Linked Site, if any. We encourage you to seek out and read the privacy policy of each website that you visit.

We may provide online forums for users who visit our site. Any information, communications, or material of any type or nature that you submit to the Forum or any of our pages contained on a Forum or Site by e-mail, posting, messaging, uploading, downloading, or otherwise (collectively, the “Submissions”) is done at your own risk and without any expectation of privacy. We cannot control the actions of other users of the Site or any Forum and are therefore not responsible for any Submissions contained on a Forum.

We may from time to time survey visitors on our site. Participation in these surveys is completely voluntary; you have a choice about whether or not to disclose information required to enter. The information collected from these surveys is used in aggregate form to help us improve our site.
