About Scott A. Hammer, MD, FAAFP
Family medicine is about families. My desire as your family physician is to provide you with the same type of personalized healthcare that I expect for my own family. As an MDVIP-affiliated primary care doctor, I have the time to focus on your healthcare needs and those of your loved ones. By providing a warm, unhurried atmosphere, I have the energy to serve as your primary care physician in a manner that you deserve. It will be an honor, as your personal doctor, to develop the type of relationship that fosters excellent health and well-being across the life spectrum.
Apart from obstetrics, I practice the full scope of family medicine, from birth through the type of end-of-life decisions that my additional board certification in Hospice and Palliative Care necessitate. I believe in quality of life throughout an entire lifetime and will work as a team with you and yours to ensure that you are living your ideal life. My MDVIP-affiliated practice offers services, including comprehensive, advanced health screenings and diagnostic tests, that go far beyond those found in concierge medicine practices. It is my fervent desire to know my patients well and, by doing so, be better equipped to manage their health.
I have special interests in pediatric, adolescent, adult and geriatric health, chronic disease management and minor office procedures, to name a few. I suppose that doesn't really narrow it down, but in my office we will be able to focus on those interests that are important to you, with an emphasis on wellness and prevention. I am fluent in English and Spanish, a member of the Bayhealth Board of Directors and Medical Staff and serve Central and Southern Delaware and beyond!

is so approachable, so informed, so ready to go above and beyond; and does. That he had a sense of humor; and allows me to display mine.
He set me up on a schedule to be seen regularly before he joined MDVIP. I'm one of those who rarely thinks I'm sick, or has anything serious enough to bother my Doctor. Hence, by the time I've ignore the issue, exhausted all the over the counter remedies, and finally realize I need to see my Doctor, I'm in trouble.
I feel that we have a working partnership. I am not one who wants to take any type of medication, and NO narcotics. He goes to great lengths to ensure that I am informed, aware of all possible options, but respects that it is always my decision.
I respect, appreciate, and value all that Dr. Hammer does; and I'm very grateful to be his patient.

I've messaged him, and he stopped, while on vacation overseas, to message back and forth, and call in much-needed prescriptions. Together we created a wellness plan, and I lost over 80 pounds, and counting! He made room in his day to care for my injured daughter, with a crushed finger, and he is now her favorite doctor in the world. (Her own regular doctor could not or would not make time to see her. This was all during Covid, so we wanted to avoid emergency rooms.) I value him so highly that when my in-laws moved here, I recommended they see him, and he now cares for them. I trust this man with my family. I cannot give him higher praise.
I feel I'm in the best health I have been in for years. Arthritis aside I'm a different person because of this practice , thanks 2 everyone.

I told Dr. Hammer what the problem was (severe swelling in the left hand, wrist and arm along with pain). I took some photos with my cell phone and sent those along to help Dr. Hammer get a better idea of what was happening.
After viewing the photos and asking a few questions regarding what Josephine had for dinner the night before. Dr. Hammerasked what drug stores were in the immediate are of Arlington, Virginia. I picked up the two prescriptions from a nearby CVS. Later that eveing Dr. Hammer wanted more photos of Josephine’s left hand, wrist and arm. Oh! Dr. Hammer also requested that Josephine make an appointment ASAP once she returned to Delaware.
After this experience there was no doubt the MD VIP program was worth the investment.

Doctors who operate in the MDVIP models typically see far fewer patients than traditional primary care doctors. That gives them more time to develop deeper doctor-patient relationships that can lead to better outcomes. They can also offer conveniences that most primary care doctors can’t, including same- and next-day appointments. They’re available after hours for urgent matters. And they have time to focus on prevention. That focus begins with the MDVIP Wellness Program, which patients pay for with their annual membership fee.
Most MDVIP-affiliated primary care practices accept insurance (your physician can tell you whether they accept your specific insurance plan). Your annual fee pays for preventive care medical services that insurance usually doesn’t cover (e.g., advanced diagnostic testing and screenings). Your MDVIP-affiliated physician will continue to bill your insurance and charge copays, co-insurances and deductibles as he or she currently does for other medical services like sick visits.
MDVIP makes wellness and prevention easier by providing your physician with innovative tools and technology. MDVIP supports your physician’s practice and seeks to eliminate barriers to a positive healthcare experience. Your doctor will determine whether and how to incorporate these tools at his/her discretion.
