MDVIP Privacy Policy

This Privacy Notice ("Privacy Notice"), describes what information MDVIP, LLC ("MDVIP", "we", "us" or "our") collects from or about you, including how we collect, use, and disclose that information, and what choices you may have regarding your information.

This Privacy Notice applies to information we collect when you interact with us, including through our website, application, social media, or other online offering (the "Website").
MDVIP contracts with healthcare providers to provide a personalized healthcare experience to their patients. MDVIP acts as a business associate when healthcare providers engage us for these services. When we process information on behalf of a healthcare provider in the course of these services, our processing is governed by HIPAA (or other applicable law regulating the handling of individually identifiable health information) and our agreement with the healthcare provider, and this Privacy Notice does not apply to that information. If you have any questions about how information is processed in those circumstances, please refer to your healthcare provider's notice of privacy practices or contact them directly.
Please note that this Privacy Notice does not apply to non-MDVIP websites or services that state they are offered under a different privacy policy. Please review the privacy policies on those websites and services directly to understand their privacy practices.
2. INFORMATION THAT WE COLLECT
Depending on your interaction with MDVIP, we may collect personal information as follows:
Directly from you, when you provide information to us or interact with us:
- Contact information, including your name, telephone number, email address, postal address (or in some cases, just your city or zip code), and any other information you choose to include when you communicate with us.
- Demographic information, including your date of birth and sex.
If you make a purchase from us, we work with a third-party payment processor to process your payment information.
Automatically from you, including device information and information about how you utilize our Website, as described in Section 6.
From other sources, including third-party data enrichment providers, to better understand our consumers and improve our services. These third-party providers provide scores, predictions, or other data points about your interests and preferences in addition to demographic information, such as household composition and geographic location.

MDVIP may use personal information we collect to:

4. TARGETED ADVERTISING AND ANALYTICS
We engage others to provide analytics services, serve advertisements, and perform related services across the web and in mobile applications. To do this, MDVIP and these partners may use cookies, pixels, SDKs, and similar technologies to collect information about your use of our services and other websites and mobile apps, including your device identifiers, IP address, web browser and mobile network information, pages viewed, time spent on pages or in mobile apps, links clicked, order information, items added to cart, and conversion information. This information is used to deliver advertising targeted to you on other companies' websites and mobile apps, understand the effectiveness of this advertising, analyze and track data, determine the popularity of certain content, and better understand your activity.
Depending on where you reside, you can opt out of ad targeting through cookies and similar technologies on our Website by following the prompts behind the Your Privacy Choices link in the site footer. MDVIP maintains internal logs for Internet Protocol (IP) addresses. An IP address is a number that is automatically assigned to your computer each time you access the Internet. MDVIP collects IP addresses in order to conduct system administration, report aggregate information, conduct site analysis, and to detect improper use of the site.

MDVIP implements administrative, technical, and physical safeguards to protect your information. While we use these measures to safeguard your information, we cannot guarantee the security of the networks, systems, services, devices, and databases we operate or that are operated on our behalf.
9. YOUR CHOICES ABOUT YOUR PERSONAL INFORMATION
Opt-Out Request of Specific Phone, Email, Text, or Marketing Offers:
If you do not wish to receive phone, mail, or email marketing offers, you may request that we remove your name from our internal email, direct mail, or internal telemarketing lists. You can call 1-866-696-3847 to make your opt-out request.

Authorized Agents

If you reside in California, you may also designate an authorized agent to submit an access, deletion, or correction request on your behalf. We may ask authorized agents to submit proof of their authority to make a request, such as a valid power of attorney or proof that they have signed permission from the consumer who is the subject of the request.
