About Gilberto Sustache, Jr. MD
Welcome to Sustache Primary Care in Marlborough, MA, where my focus is on your wellness. I offer a more individualized approach to healthcare with the MDVIP model. My aim is to put your health first while encouraging balance and self-care. With over 20 years in primary care, I can focus my education, experience and enthusiasm on you.
My MDVIP-affiliated practice offers services, including comprehensive, advanced health screenings and diagnostic tests, that go far beyond those found in concierge medicine practices. My practice size is smaller, which means I can offer conveniences like same- or next-day appointments with minimal waiting. It also means I have more time to spend with each patient to discuss primary care issues big and small.
If I’m privileged enough to meet you, I hope you’ll see that I love people. I am happily married for over 27 years and have three amazing kids. I love detailing cars for fun, and I am fluent in Spanish.
I hope you’ll feel encouraged to come and find out what my practice is all about. I appreciate you taking the time to look me up.

Reviews from the web
Early one weekend morning our daughter became very sick. My wife called Dr. Gil and he answered immediately. During that call her condition quickly pivoted from a situation where my wife and I were going to drive her to a nearby emergency department — to needing an ambulance. Dr. Gil calmly walked us through calling EMS, told us exactly what to communicate to the EMTs, and explained what to expect at the ED. As a father I felt overwhelmed and powerless. Dr. Gil proactively checked in by phone and text and stayed reachable while we were at the hospital, which kept us from allowing our fear to consume us during that very long day. His clear, practical guidance made a huge difference and I was deeply grateful to have direct access to him in that moment.
Beyond that emergency, Dr. Gil has changed how I think about health care as a man. I grew up believing men should “tough it out,” and asking for help felt like weakness — a mindset reinforced by past experiences with other providers who were dismissive, rushed, or minimized my symptoms. Dr. Gil created a nonjudgmental space where I can be honest about my health concerns without shame. He listens, explains options in plain language, and involves you in decisions — which has made me see seeking care as responsible, not weak. He also takes a whole-body approach, addressing both physical symptoms and mental/emotional well‑being, and considers how stress, mood, and lifestyle affect overall health when tailoring care.
If you’re a man who’s been hesitant to seek care because of stigma or pride, Dr. Gil will put you at ease. His combination of calm leadership in emergencies, everyday compassion, and proactive advocacy is rare — and it’s why I trust him and recommend him to other families.
Appointments are scheduled so you’re walked right in with no wait — I’ve never even seen another person in the office at the same time. They take confidentiality seriously, handling conversations and records discreetly so appointments feel private and respectful. I hope his practice continues to grow — not only because Dr. Gil is an exceptional person and clinician, but so that more men raised with the “tough it out” mindset can experience what men’s healthcare should be: respectful, private, and proactive.
He is a fierce advocate for his patients — fighting with insurance companies to secure needed testing and personally calling outside providers to coordinate care and advocate for the best overall outcome. His attention to detail and commitment to getting accurate answers is unmatched, and his passion for patient care is reflected in the way he runs his practice.
Access to Dr. Sustache is outstanding. He provides direct lines of communication — phone, text, and email — so medical questions and concerns can be handled efficiently. He never dismisses questions or makes you feel foolish; patients are treated like family. As an added bonus, he has a warm sense of humor that helps put you at ease.
He truly goes above and beyond — even consulting with close relatives who may be visiting for the holidays or vacation (not his patients), taking the time to offer practical advice and help coordinate next steps when needed. That willingness to help in informal situations speaks volumes about his character.
His wife also works at the practice and is incredibly kind and helpful, which adds to the welcoming, well-organized atmosphere. I highly recommend Dr. Gil Sustache to anyone seeking a compassionate, ethical, and skilled doctor who truly advocates for his patients.
Now in Spanish: “Excelente, profesional, atento y amable son pocas de la muchas palabras en las que pienso tras nuestra visita. Gracias por tomarse su tiempo para comprender mis inquietudes y por proveerme tu atención. Te lo agradezco!
Frequently Asked Questions
Doctors who operate in the MDVIP models typically see far fewer patients than traditional primary care doctors. That gives them more time to develop deeper doctor-patient relationships that can lead to better outcomes. They can also offer conveniences that most primary care doctors can’t, including same- and next-day appointments. They’re available after hours for urgent matters. And they have time to focus on prevention. That focus begins with the MDVIP Wellness Program, which patients pay for with their annual membership fee.
Most MDVIP-affiliated primary care practices accept insurance (your physician can tell you whether they accept your specific insurance plan). Your annual fee pays for preventive care medical services that insurance usually doesn’t cover (e.g., advanced diagnostic testing and screenings). Your MDVIP-affiliated physician will continue to bill your insurance and charge copays, co-insurances and deductibles as he or she currently does for other medical services like sick visits.
MDVIP makes wellness and prevention easier by providing your physician with innovative tools and technology. MDVIP supports your physician’s practice and seeks to eliminate barriers to a positive healthcare experience. Your doctor will determine whether and how to incorporate these tools at his/her discretion.
