About Scott A. Hammer, MD, FAAFP
Family medicine is about families. My desire as your family physician is to provide you with the same type of personalized healthcare that I expect for my own family. As an MDVIP-affiliated primary care doctor, I have the time to focus on your healthcare needs and those of your loved ones. By providing a warm, unhurried atmosphere, I have the energy to serve as your primary care physician in a manner that you deserve. It will be an honor, as your personal doctor, to develop the type of relationship that fosters excellent health and well-being across the life spectrum.
Apart from obstetrics, I practice the full scope of family medicine, from birth through the type of end-of-life decisions that my additional board certification in Hospice and Palliative Care necessitate. I believe in quality of life throughout an entire lifetime and will work as a team with you and yours to ensure that you are living your ideal life. My MDVIP-affiliated practice offers services, including comprehensive, advanced health screenings and diagnostic tests, that go far beyond those found in concierge medicine practices. It is my fervent desire to know my patients well and, by doing so, be better equipped to manage their health.
I have special interests in pediatric, adolescent, adult and geriatric health, chronic disease management and minor office procedures, to name a few. I suppose that doesn't really narrow it down, but in my office we will be able to focus on those interests that are important to you, with an emphasis on wellness and prevention. I am fluent in English and Spanish, a member of the Bayhealth Board of Directors and Medical Staff and serve Central and Southern Delaware and beyond!

My doctor is like family to me. I can't imagine not having him in my life.He has been through so many life changes with me. I can always count on him. I feel he was the reason my husband lived a little longer than he would have if Dr. Hammer hadn't seen something suspicious in a MRI. He is just all around BEST !!

An early June 2023 I went to Dr. Hammer‘s office along with my brother complaining of how my belly felt and he sent me to Bayhealth to get a CAT scan. The results showed that I had an area of scar tissue which led to a small bowel blockage. The blockage though caused by the scar tissue they were unsure on how it even developed and it ended up causing Pierre appendicitis. I ended up in and out of…

Scott has always been proactive in his approach to my medical care. I have undergone a cervical and a lumbar fusion after he referred me to a major hospital with an excellent reputation. He followed me after my return home, in addition to the followup at the hospital where the the surgeries were performed.
Additionally I was hospitalized with double pneumonia in 2023 at a local hospital with hospitalists. Scott checked in with my by text daily which showed he was monitoring my labs and xrays. That was very comforting to me to know he was being active in my care. He followed up with x-rays and CT scans after I was discharged until we were sure the pneumonia was completely gone. We jointly decided, after my discharge, that I should be followed by a pulmonologist for a mild lung disease and as possibly preventing further pulmonary crisis. He made a referral and the two communicate with each other.
I trust Scott's medical judgement completely and feel so confident with any referral to a specialist that he makes. I also "like" him, feel respected and valued by him. I never feel judged and am comfortable discussing any issue or concern I have with him. I told Scott several months ago that going to an appointment with him is like going to visit a favorite nephew.
His staff is extremely efficient and welcoming. The office is pleasant and staff follow through with a request if one is made.
I cannot imagine having to use any other primary care physician and feeling the amount of trust and confidence that I have in Scott and his staff.

As some one else posted, I, too, transitioned into MDVIP care because Dr. Hammer did.
He has always given fantastic care and taken the time necessary to do it. Now he does not have to worry about the overscheduling that used to be necessarily commonplace. (Nor do I. )
I know not everyone can afford this plan. I am grateful that my husband and I make it a priority. Our health and peace of mind are worth it.

Dr. Hammer is a personable, intelligent, well informed and caring physician. He is quick to respond, even by text message, after hours. He was notified when I was admitted to the local ER, and called my husband to check on me. I can't say I have ever had a phjysician do that before. If you are looking for a doc who is responsive, this is your guy.

Our physician, Scott Hammer truly cares about the health of our family. He always answers our calls/texts and follows through with us to make sure we feel good about our health situations and decisions. We are always able to get an appointment with him when needed and he has been super responsive on off hours in emergencies. He’s also amazing with our children and wants them to be healthy and happy. During their visits he spends time getting to know them and talking with them about who they are and how they are growing. My husband and I have had a lot of medical issues over the last few years, and I wouldn’t have been comfortable with anyone other than Scott working with us to make sure our health needs were taken care of. He’s attentive, listens and provides good advice. He also makes sure we are referred to the best specialists when needed. We are so happy that we made the switch to the MDVIP program and have Scott as part of our family.

I am extremely pleased with Dr Hammer. How many times can you call the Doctors office and get in immediately. Because of my age I’m on Medicare and he spends 2 hours twice a year going over the thorough tests results. Myself and Doc set goals to improve my health. Couldn’t ask for better service.

I am extremely happy to be a patient in Dr. Hammer's practice. I wish that I had joined sooner. The quality of the health care that I am receiving is superb. He has helped me several times when I became ill while traveling. He is reachable 24 hours a day and for that I am very grateful. I was recently hospitalized and he was "on top" of my treatment and the care I was receiving. He communicated several times daily with my husband to reassure him that everything necessary was being done to help me. About a year ago, I called him when my husband became ill while traveling and Dr Hammer was able to provide him with treatment so that we could safely return home. Great Doc and great staff! I am 80 years old. I have had many doctors. Some OK some so so. Moved to Delaware 20 years ago. After 5 years met Dr. Hammer. Never thought there was one as dedicated as he.

He set me up on a schedule to be seen regularly before he joined MDVIP. I'm one of those who rarely thinks I'm sick, or has anything serious enough to bother my Doctor. Hence, by the time I've ignore the issue, exhausted all the over the counter remedies, and finally realize I need to see my Doctor, I'm in trouble.
I feel that we have a working partnership. I am not one who wants to take any type of medication, and NO narcotics. He goes to great lengths to ensure that I am informed, aware of all possible options, but respects that it is always my decision.
I respect, appreciate, and value all that Dr. Hammer does; and I'm very grateful to be his patient.

Before I became a patient of Dr. Hammer I had very little confidence in doctors. My experiences in the pass left me not wanting to go to the doctors. Knowing I needed a doctor I could believe and trust I did the research and found the Delaware Southern Medical Group, specifically, Dr. Scott Hammer. He listens to what I have to say. He explains everything to my satisfaction. He takes an interest in my overall health. Makes recommendations that makes me into a well rounded person. He is, in my opinion, what a doctor should be for everybody.

Scott really cares about his patients. He listens to what they have to say. He doesn't just tell you what you want to hear, he tells you what you need to hear. He takes his time with you. He is always pleasant and serious when he needs to be. And I really have to hand it to you Doc... You won over my husband and that is not an easy task. He thinks you walk on water. Most MDVIP-affiliated primary care practices accept insurance (your physician can tell you whether they accept your specific insurance plan). Your annual fee pays for preventive care medical services that insurance usually doesn’t cover (e.g., advanced diagnostic testing and screenings). Your MDVIP-affiliated physician will continue to bill your insurance and charge copays, co-insurances and deductibles as he or she currently does for other medical services like sick visits.
