About Lillian E. Cohn, MD, FACP
I am a female primary care physician with over 30 years of experience in internal medicine. I also have the privilege of providing personalized health and wellness care in a model. My office is in Center City, Philadelphia, and my patients come from the southeastern Pennsylvania region; New Jersey, as far away as Trenton and Cape May; Delaware, including Rehoboth; and even Maryland. I am on staff at both Pennsylvania Hospital and Thomas Jefferson University Hospital and continue to teach residents and students as part of my commitment to educating the next generation of primary care physicians.
My goal as an internist is to partner with my patients so they can become better advocates for their healthcare needs and participate more in the medical decisions that can help them live healthier and happier lives. I manage both acute and chronic diseases and assist with end-of-life decisions. Because of my smaller practice size, I have time to closely listen to my patients and provide proactive medical recommendations and emotional support. My MDVIP-affiliated practice also offers services, including comprehensive, advanced health screenings and diagnostic tests, that go far beyond those found in concierge medicine practices. My greatest asset is my ability to advocate for my patients' needs, and I am available for all of their questions and concerns. As a private doctor, I coordinate care through our complex healthcare system, and in so doing, I am able to help improve the overall wellness of my patients and limit unnecessary services. I have an honest and direct approach to communication and am sensitive to the different needs and goals of individuals.
My MDVIP-affiliated internal medicine practice has earned the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Level III Certification and has repeatedly achieved above 95% nationwide for quality. This recognition acknowledges an effective way of organizing primary care that emphasizes care coordination and communication in order to transform primary care into "what patients want it to be."
