After my husband died nearly five years ago, eating my favorite things was a comfort. What wasn't comforting was the 20 odd pounds I put on in the following years. With the encouragement of Dr. Hammer who told me about the diet meal plans for 1 on the MDVIP website, I was able to stick with a diet during the lockdown and subsequent weeks of the pandemic and I have lost that 20 pounds. His… See more
My Philosophy and Approach to Wellness...
Apart from obstetrics, I practice the full scope of family medicine, from birth through the type of end-of-life decisions that my additional board certification in Hospice and Palliative Care necessitate. I believe in quality of life throughout an entire lifetime and will work as a team with you and yours to ensure that you are living your ideal life. My MDVIP-affiliated practice offers services, including comprehensive, advanced health screenings and diagnostic tests, that go far beyond those found in concierge medicine practices. It is my fervent desire to know my patients well and, by doing so, be better equipped to manage their health.
I have special interests in pediatric, adolescent, adult and geriatric health, chronic disease management and minor office procedures, to name a few. I suppose that doesn't really narrow it down, but in my office we will be able to focus on those interests that are important to you, with an emphasis on wellness and prevention. I am fluent in English and Spanish, a member of the Bayhealth Board of Directors and Medical Staff and serve Central and Southern Delaware and beyond!
I look forward to our future together.

Testimonials from patients of Dr. Hammer
My wife and I have been patients of Dr. Scott Hammer in Milford, DE for over 10 years. First impressions are always very important. To many of Dr. Hammers patients this is given with a smile by Kandace Rowe, a long time staff member who is the perfect person for this position. From your first visit on you know you are being cared for by a professional. She is attentive to your needs for… See more
My husband and I have had Dr Hammer as our physician for over 10 years. We are so blessed that he was recommended to us by a friend. Dr Hammer you are the most warm and caring person I know. I have never had a physician like you who has spent the time to answer any questions and to make sure I understood the information given. You have given us options/choices for our medical issues/treatments… See more
My husband was recently diagnosed with Melanoma. We were in no way prepared for this diagnosis; nor prepared to process it Dr. Hammer was invaluable assisting us with making needed decisions in a timely manner. He ensured we had a PET scan done, were connected to an Oncology team at Penn Medical, and Surgeon specializing in this specific type of cancer. In approximately 6 weeks of our initial… See more
Last year I was traveling in Germany and developed Bells Palsy. Being able to reach Dr. Hammer directly meant so much to me as he was able to diagnose and confirm what the doctor in Germany thought was going on and advise me what to do. What a comfort that was! And upon returning home he came to my home to check me out and make sure I was OK. The personal care I've received from him… See more
Dr. Hammer is a wonderful Physician! His care and sensitivity and diagnostic skills are beyond amazing!
The day after Thanksgiving I cut my finger and instead of going to the ED I texted Dr. Hammer a picture and he immediately instructed us to meet him in his office where he stitched my finger! No questions asked—only concern and care!
Dr. Hammer consistently treats us as we are… See more
I was dealing with scratchy pain in my eye for about three years, that was getting worse. I saw three different eye doctors. The last one suggested dry eye drops which I followed for two weeks, but still getting worse. When my husband and I signed up for MDVIP with our longtime family physician Dr. Hammer, I finally brought up the eye problem, since MDVIP included an eye exam. I also told Dr.… See more
Dr. Hammer has attended both my wife and I, her father and mother, my father and her sister. When circumstances required he has made several calls at our home, mostly in situations where elderly family members were in need of either medical skills or just comfort when end of life was near. His compassion, skill, bedside manner and wonderful personality have made him "OUR DOC". As my… See more
Once upon a long time ago, my 94-year old father-in-law (fighting metastatic colon cancer) and my nearly 80-year old mother (fighting Progressive Supranuclear Palsy) both needed doctors who could help them remain comfortable through essentially what was end of life.
Enter Dr. Hammer. He was a comparatively young doctor with a ponytail. Neither parent had ever had exams so thorough… See more
I originally started seeing Dr. Hammer when my previous doctor had my very anxious that I might have a tumor on my pancreas -- an extremely unlikely condition that the data did not support. He helped me review the large amount of blood sugar data, devices I was using, and realistic talk about blood sugar patterns for type 2 diabetics that helped me regain my logic and wisdom about my current… See more
